Explain the importance of a “spirit of inquiry” in an evidence-based culture

As step 0 of the EBP process, a spirit of inquiry refers to an ongoing curiosity about the best evidence to guide clinical decision making. When nurses ponder and collaboratively improve clinical problems using clinical reasoning and judgement, a spirit of inquiry exists and the first step of the EBP process can begin (Wyant, 2018). The best way to encourage this within a practice is to never be satisfied with the current practice. As nurses, we should always be looking at what improvements can be made and how we can better care for our patients. In the emergency setting, EBP is used to maintain and improve best practice in regard to efficiency in emergency medicine. Meaning, stroke interventions have time limits and goals. These time goals are applied to STEMIs, strokes, sepsis alerts, pain management of fractures, treat and discharge, treat and admit, etc. These goals are set by EBP guidelines and set a standard and expectation of patient care.

A spirit of inquiry by nurses that goes through a systematic process for raising clinical questions is a core factor in EBP journey (Wyant, 2020). This occurs when a nurse thinks of a question in relation to an existing clinical practice and seeks ways to improve that particular practice. One way to encourage this within the organization as an advanced practice nurse is to lead by example. For example, while working in the unit we see multiple elderly patients having difficulty sleeping in the hospital, our thinking minds as clinicians will ask ourselves how we can improve their sleep patterns. This process is called the spirit of inquiry, where the “questioning” begins to exist. Motivating staff to share their experiences and thoughts should be encouraged. Their curiosity could potentially lead to improving patient’s health.

The role of EBP in my specialty area of practice with emphasis in healthcare quality and patient safety is significant in improving clinical practice and addressing patient safety concerns. Because EBP involves a problem-solving approach that integrates research evidence, it can improve health outcomes and quality of service (Songur et al., 2018). In comparison to other specialty practice, such as nurse anesthetists, EBP can help them determine which interventions can assist in guiding their practice. For example, if there are high rates of atelectasis, they can come up with EBPs that focus on the role of breathing exercises to be used pre and post procedures to determine if such interventions can help in decreasing incidence rates of atelectasis in patients. It is important to have a spirit of inquiry culture to be successful in EBP (Evidence Based Practice). A spirit of inquiry within an EBP culture is a culture that welcomes questioning. Clinicians should feel comfortable and encouraged to consistently question the “why” behind their practice. If clinicians are excited and comfortable asking questions, they will be more inclined to challenge current practices. Successful organizations include EBP in their philosophy and mission. Advanced Practice Nurses play a vital role in implementing EBP by serving as mentors (Melnyk & Fineout-Overholt, 2019). In my current role as Nurse Manager, we incorporate EBP in all our unit-based council projects. It is essential that leaders create the vision for EBP, provide resources to support it and incorporate evidence into practices (Liu, 2021). Many clinicians feel EBP searching and appraising articles is overwhelming and time consuming. It is important to educate them on how to access Cochrane Library and the Guidelines International Network (Liu, 2021). All EBP projects must have a PICOT statement. All members of the unit-based council are encouraged to collect and share relevant best evidence. We also have a Research Nursing Council that can support the unit-based council with their projects. These EBP mentors are important to overcome barriers (Melnyk & Fineout-Overholt, 2019).

Spirit of inquiry is defined as a continuous sense of wonder that guides both learning and practices. Nurses can be highly influenced by the spirit of inquiry practices in a beneficial way. In nursing, the spirit of inquiry will raise questions, challenge established and existing practices, and look for new ways to solve problems. When asking the right questions in the right way, obtaining the best available evidence, and determining what practice changes are needed are all part of addressing a clinical problem through evidence-based practice (EBP) (National League for Nursing, 2021). Developing nurses with an inquiry mindset within a culture that promotes a systematic approach to asking clinical questions is a critical component of the EBP journey. When conducting an evidence-based study or question, utilizing the spirit of inquiry supports the ongoing curiosity of evidence that would dictate a clinical decision. As a nurse, it is essential to understand if an intervention is necessary, beneficial for the client, and supported by evidence-based practice. Nurses and collaborative team members work to improve clinical problems by using clinical reasoning or judgment. The spirit of increased exit and the first step of the evidence-based process will begin. The overall outcome or goal in predictable patient care settings, nurses will use evidence, tradition, and patient preferences in collaboration with health care team members to promote optimal health status.
