Identification of Clinical Problem or Issue

Choose a mortality/morbidity indicator to identify a clinical problem or issue that you want to explore pertaining to a population of focus. Every evidence-based practice project begins with a problem to be solved. 

The problem statement presents the idea, issue, or situation that you intend to solve through application of evidence-based practice. The problem statement is the foundation of the project and is usually preceded by several paragraphs of background information that set the stage for the proposed project. 

These paragraphs identify the significance of the problem, present justification that the problem actually is a problem and is researchable and provide supporting documentation from the literature. You must always include the population of interest and the variable(s) or outcomes to be measured. Use this indicator to begin to formulate a PICOT and conduct research on the population.

In African Americans (P), what is the effect of a community-based physical activity and nutrition education program (I) on the reduction of diabetes incidences (O) compared with no intervention (c) within 6 months (T)?

Demographics

The African Americans were about 19% of the total population in 1790. At the beginning of the civil war, in 1860, the number of African American increased to 4.4 million. Most of them were still slaves. Only 488,000 of them counted as freemen (HHS.gov, 2019). The number of Blacks in America has been increasing rapidly over the past several years and now makes the second-largest minority groups in the US after Latino.  In 2017, African Americans were 41.4 million people in the United States which represents 12.7 % of the total population. Most African Americans (58%) lived in the South (HHS.gov, 2019). Also in 2017, the African Americans median household income stood at $40,165, bellow that for Whites.

Health Concern of the Population

The most common kind of diabetes is type 2 diabetes, which affects a high number of African Americans. African Americans, according to the National Institutes of Health (2018), are more likely than other populations to suffer from the ailment. African Americans are 77% more likely to develop diabetes than Whites. Diabetes can lead to major health issues such as heart disease, coronary artery disease, and stroke (National Institutes of Health, 2018). Neuropathy, nephropathy, retinopathy, foot damage, skin disorders, depression, and Alzheimer’s disease are all typical complications in this population.

Nursing science, health determinants, epidemiologic data, and genetic data may all have an impact on African American diabetes care. Diabetes management treatments such as nutrition and physical activity, as well as appropriate drugs, have been established by nursing science (National Institutes of Health, 2018). Diabetes research is still happening in attempt to enhance diabetics’ lives. All of these therapies, including education and counseling, are supported by nursing science.

Health determinants can provide insight into the underlying cause as well as the necessary management to improve community health.

People’s behavior or genetic make-up, for example, can help explain why type 2 diabetes is becoming more prevalent in the population (National Institutes of Health, 2018). Epidemiological data can also be utilized to evaluate the prevalence of diabetes among African Americans. Because type 2 diabetes risk factors can be inherited, genomic and genetic data are crucial. Variations in the HLA-DQA1, HLA-DQB1, and HLA-DRB1 genes are also associated with an increased risk of developing type 1 diabetes (National Institutes of Health, 2018). The findings of these health determinants can be used to improve community health.

PICOT (Population/Problem, Intervention, Comparison, Outcome, and Time to Achieve the Outcome) is a strategy for determining the characteristics needed to develop a good question from a practice issue or problem affecting the population of interest. Furthermore, the information obtained from a good PICOT allows doing a literature search to discover translational research sources that can be used to treat the clinical problem easier.

Consult a national, state, or local population health care database to explore inequities in population health care. In a paper of 1,250-1,500 words, synthesize the research into a literature review. The literature review should provide an overview for the reader that illustrates the research related to your particular PICOT. Include the following:

Introduction: Describe the clinical issue or problem you are addressing.

Methods: Describe the criteria you used in choosing your articles

Synthesize the Literature: Part A: Discuss the main components of each article (subjects, methods, key findings) and provide rationale for how this supports your PICOT; Part B: Compare and contrast the articles: Discuss limitations, controversies, and similarities/differences of the studies.

Areas of Further Study: Analyze the evidence presented in your articles to identify what is known, unknown, and requires further study.

You are required to cite five to 10 sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.
